I'm not novita but you need a copy of the passport including the residence permit (or a copy of the Qatari ID), 2 photos, fill in the form and 100 Riyals per applicant.
I also know that you can (theoretically) do it online at the e-gov site but I don't know how they handle the photos.
Applications can be gotten from Hamad Hospital in the Outpatient department, Rumailah Hospital at the cashier or any of the health centers.
