Were lessons learnt following a similar incident in another country? With hind sight, this tragic loss could have been prevented.

Someone will no doubt investigate the matter to establish exactly what happened on the day. Then and only then will lessons be learnt.

Most risks are adequately controlled by implementing a comprehensive Health & Safety system. Risk Assessments enable one to anticipate what could go wrong and to put controls in place to minimise or completely eliminate the likelihood of it going wrong. Is this a requirement on all schools?
