I can understand ur worry,, however there shouldnt be actual cause to worry if the kid stops the habit before the age of atleast 5 years. normally it should go before the age of 3.
now if all the techniques u mentioned dint work, then iam pretty sure tyingup the elbow will also not help..or atleast u can give it a try. I've come across two parents who literary tied up the kid's elbow with kind of plasters or films that don't let the child turn his hand to reach mouth. Now again, that will surely need kid's cooperation. Did u try speaking to ur kiddo convincing him about the bad effects when he growup?!
anways,, if all of this dosen't workout,,u will have to visit an orthodontist inorder to fix a crib on child's teeth. This is said to be very effective to stop thumbsucking. Any orthodontist can help you with better advise than I can. :) try speaking to one.
