Approach one or more muslim friends, ask them to guide you saying the declaration of shahada (There is no god but Allah and Muhammad is Allah's Messenger).
That's it, voila you are a Muslim.....
If you want a more 'formal' process then, as many had suggested, go to Al Fanar near Souq Waqif and Al Ghanim Bus station. They have an excellent program to help new (and old) Muslims undertanding their faith. I would imagine they will also be able to help you for things such as changing your name to muslim ones (not compulsary) etc.

The starting point is to pronounce the Shahaadatayn (Declaration of Faith).
The Shahadah can be declared as follows:
"ASH-HADU ANLA ELAHA ILLA-ALLAH WA ASH-HADU ANNA MOHAMMADAN RASUL-ALLAH".
The English translation is:
"I bear witness that there is no deity worthy to be worshiped but Allah, and I bear witness that Muhammad is His servant and messenger."
