I am not sure if you wrote this poem because someone dear to you is leaving. If that is indeed the case, an open heart to heart talk is the only answer.
Its always one-on-one dialogue that helps, i believe so. And dont try calling her on phone, insist that you need to SEE her. E-mails, phone calls can never substitute for your presence. And the presence in front of her when you talk to her is very important.
I dont know what her reason for leaving is, but if it indeed is something that cannot be repaired, then please, dont prolong the heartburn. There is an old saying that 'if you love a person, let her go and if she returns, she will never leave again'.
Maybe, it may not be that bad after all. Maybe she just needs a break. But whatever it is, try and respect her decision. She will much appreciate that and most possibly (knowing the wonderful guy you are), will return to you. Be optimistic, chin up. You are not gonna let that broad shoulder sag.
It will take more than this to beat you down, my man. This is but temporary. The tough part is to not go under, and i fully believe you are capable.
On another note, the poem was brilliant. Even if the underlying mood was sad, it came out with exactly what you were feeling. And that, is the hallmark of a good poem. Well done. Hope your girl reads it as well...
