Bungee cord snaps
Australian thrillseeker Erin Langworthy followed thousands of others over the past decade when she bungee jumped off a bridge across the Zambezi river at Victoria Falls in Africa. But the cord snapped while she was still at least 20 metres above the water, sending her on a potentially deadly plunge with the remains of the cord binding her ankles. She managed to swim to safety and was seriously bruised and scratched but had no broken bones

Regardless of the risks, if the cord snapped it is surely the negligence of the owner.
Anyway, glad she survived that.
