Medical for Visa

Dear all,

I have my family here in RP Visa and I am planning to call my wife's sister and her 3yr old son in visit visa. I know i can call my wife's sister, but can i call her son also. As i have heard that i can apply visit visa only to direct relations like wife's father, mother, sister or brother. But in my case her son is too small to be alone with her relatives and need to accompany with his mother.

Also If they are coming, To do medical for Visa, after their arrival, do i need to do Medical for that 3 yr old kid also.?
