Medical center close to Al- Sadd for Hamad health card

Hi everyone, I am living at Al-Sadd area near sports round about, which medical center I need to go for Hamad medical card for family? What is the process and document I need for Hamad medical card for family? If anyone know, please response.

it is based on your house location mentioned in the house contract. if it is mentioned

al sadd area then you have to visit mesaimeer health center.

documents QID, passport copy with visa page, house contract & latest Water & Elec Bill,2 photos. and keep said all original documents to show to them.

you should be there in the morning 07:00 AM SUN - THURSDAY.
