Sheraton Health Club and Beach is still the best in Doha.. It might be one of the oldest but very well maintained and they have the best beaches amongst the hotels.  The gym is the best compared to all I have seen so far and they have spinning classes

Choose according to location of accom or work place as otherwise heavy traffic may de motivate you in the evenings.

Most hotels have Gym/spa/and some have beach etc.

Golds Gymn is on D ring road.

Cost varies from Garveys (European club) around QR4500 pa to I think around QR18k for 4 seasons/intercon hotels etc. Marriot/sheraton between this range. Best to invest some time when you arrive and see which"feels" right for you and does not have waiting list. Buy in haste- repent at leisure!

I like Al Dana... indoor/outdoor pools, tennis, gyms (2),restaurants, ladies hours and more...

10,000QR a year, pay up front... This is where all the clubs get you, they keep raising their prices! This is about the average for a nice one, and you have to fork it over before you even start!
