how to complain against a restaurant to baladiya

last day i went to a restaurant near by my villa along with 4 other friends and after eating their food all 4 of us are having severe stomach pain and diarrhoea..The resturant looked neat and clean in serving area..but who knows about inside the kitchen..and the chef inside...i want to give a complaint against them to baladdiya(heath dep.)..what should i do?
