Go to National Health Authority Office (NHA). Fill up the form and get the Birth Certificate. If you already have the Certificate, Go to your Embassy, apply for baby's passport. Then apply for visa.

According to my knowledge and experiences, the penalty for keeping the baby without proper docs. is QR.250/day. I paid QR.2500 for 10 days @ 250/day in 2007. I was not aware of the procedures. So Just took the passport and went for vacation with the family. while exiting, immigration guys stopped me at the counter saying that, the baby has no visa. So they penalized me.

Note: I think the mother can keep the baby here for 2years if father is not in Qatar. She has to endorse the baby's name on her passport.

Consult a good Mandoop and get the latest rules and regulations...
