Dear, Take a copy of your visa, Passport Copy, Photo, Visa Extension Form ( you can download, type and print from www.moi.gov.qa or go to typist in Souq Waqif). After complete the all documents, go to Medical Commison in front of Waqood Petrol station near abuhamour, with in hour your medical will be done.

passport
ID picture
QR 100
Go to typing room at NHA for the form.
