I tried applying family visit visa for wife through metrash 2 app?

i tried it yesterday and it asked for my wifes passport details and name and asked to attached passport copy and marriage certificate which i did, but same times the status is now cancelled.

I attached our marriage certificate or her passport, but in the name column is there any mistake i didn't understand first name or last name. Please clarify guy if anyone applied. with through all processer
if anyone else has tried applying from here please do let me know if i am doing something wrong.
