Hello Anish....it is a very simple process you just need go to the nearest immigration dept with the birth certificate , marriage certificate ,Baby's passport & PP size photo (with blue background), Mothers and fathers passport with valid visa and take a number for visa counter and give this all documents at counter they will issued a visa for baby immediately this all process will take just 5 min only and does not required any kind of form for this whole process. As I did the same at last year for my baby as well. Hope it is clear for any one.
