ccc123: I think I met someone in the Embassy that is the same as your case.. she just have a "authorization letter" valid I.D of the person that is applying and a valid I.D of the person doing the task. I just went to my school, then asked them for "CAV" and I paid 280php. Then after 1 week they gave me a "claim stub" from the DFA office, after 1 month my papers are done.. :)
