I would suggest to look the following:

1. Smoke extraction fan/s (this is the fan to extract heavy smoke out of the building during fire). I would presume that most of the victims died of smoke suffocation including the responding firemen.

2. I agree about the impropriety regarding evacuation during fire since one comment above said that during the fire, there are portion of the mall that appeared to be "business as usual"

And again I will say, if you experienced dealing with Civil Defense regarding occupancy permit, to the least detail of requirement for fire safety, they will need to see by themselves. Of course I am talking about the last five years.
