I know a person who has the same case, his name on his educational certificates are different. He always carries with him the copy of affidavit. But if you still have time, after getting the affidavit done, better if you request your school to correct your name in your educational certificates and ID's.

You need several copies of that affidavit and NSO birth certificates to request for correction.
