I tend to avoid eating at the more 'expensive places' altogether, but not just for this reason. Other than the fact that I find the whole business model exploitative of the labour, and the food is generally not much better than the cheap places anyway.
And to be perfectly frank, it is a rare event indeed when I get service in a cafe or restaurant that's worthy of a tip. Again, the service in the cheaper, family run places is much better than it is in the supposed 'high end' dining places, such as Mint.
At most places the staff are late, inattentive, don't know the menu and typically just lie to you when you ask if something can be done in a specific way.
That being said if the service is worthy of a tip, I will never 'leave' one - you have to actually give it to the waiter otherwise it goes straight in the till.
