reqts- 1) schengen visa application form, can be downloaded at any UK embassy sites I guess, visit British embassy for example and you can download it from there + pics
2) bank statement for 3 months
4)health insurance - duration of stay there
5)accomodation i.e. hotel bookings
6)2 way airticket

complete all of these and you can submit it to the embassy.
