There are two options (1) Pls go to your health center where you have registered as an out patient and submit your application then they will issue the card in due course. (2) As you r near Gharaffa, go to Madinath Khalifa health center which is located next to Muroor (Traffic Dept) and apply it. I hope u satisfied with this information. Wish u all the best
