May I offer the following suggestions:

1) Have everyone sign a user agreement.
2) Give people 1 or 2 warnings maybe 3, but then it's 3 strikes you are out. At times we all let our emotions get the best of us and then we act before thinking so this approach will give people a chance.
2) Have moderators and assign them rights so they can take care of repeat offenders immediately.
