Bring a golden retriver dog into Qatar

Hi, I am going to move to Qatar in end of Sep.
We have a golden retriver dog and we would like to bring her with us. What kind of document I should fill in ?? Does she need to do any test??
I try to e-mail to one of the pet locator but no way reply to me. I am wondering anybody here can help me.

To import a pet you need an import permit. Your company can organize you one, it costs only QR10.

You might also need an export permit from your country (I know this holds true for South Africa). Call the airline you want to use and check with them.
