Can you please tell me how can I obtain health card if I'm on visit visa in Qatar as my husband works here ..
1. a filled up form
2. 1 photo
3. Visa copy
4. 100 riyals
Go to HMC or walk in clinic to apply. For family status visa holder, apply to your nearest health center.
first you visit the hamad health center which is near to you, they will give you a form, fill up that form and you can easliy understand what they need. everything is mentioned on that form. so fullfill that requirements and submitt to the center with 100 riyals fee. with in 15 minutes they will give you the card.
i think ID card, PHOTOS, street name and number and electricity bill copy will be required at that time as your area number is there on electricty bill.
but better if possible visit any hamad health center and collect that form so you can proceed after that.
PP & ID copies, 2 Photos, 100 QAR, one week time ...
