Health card Requirements.

Applied in JULY,2019

I am writing this to help and guide people applying for health card the documents they’ll need. 

*Your QID and passport copy

*Your sponsor’s QID and passport copy

*House agreement 

*Building no. Slate picture printout  (the clear picture of blue placard outside your building which mentions your building no. Street and zone/picture is attached for instance)

*Two blue background photographs. 

These requirements were asked at Gharrafa health centre when I applied there for health card, hiwever this information may vary when applying HC in another area.

(We faced a bit of difficulty because we didn’t know)
