1) You can pick up the form for the Health Card at Hamad.
2) You need to go to the PHC (Primary Health Care Centre) in your area.
3) You have to take a copy of your electric bill with you so that they know exactly which area you belong. (The electric bill has the area code 55, 57 59 etc.)
4) You need your Qatari ID
5) When you have all of these then go along to your local PHC and pay and apply for the Health Card.
Better to go early morning otherwise you will have to wait too long. The Administration closes at 1 pm.
Hamad had picked up on administration and have authorised the PHC's to give out the cards. Finally in the future as it has a chip you will b able to pay from a Bank Automat.

just visit HMC provide them the personal details like address, cont #.....etc, pay Qrs.100/- and that is all.
