Guidence reqiured to transfer my HMC centre?????????????

dear all,

kindly advice me regarding the transfer formalities of Hamad Healt Centre. Actually i was living in old airport area. I got HMC cards for myself and for my family memebers.
now i have been shifted to Ain khalid Area. The nearest health centre is Messamer Health centre.Now i want to transfer my health centre from old air port to Messamer. what documents are required for that.
Also my baby is getting vacination from old air port health centre, is transfer of vacination card is also reqiured for that or I can use the same vacination card at Messamer centre which we are using at old airport?

go to your health center and ask for the pink slip. tell them that you are trasferring to messameir health center. bring that pink slip to messameeir health center. you can still use your baby's vaccination card in messameeir health center.
