Medical card and related process when visting

Quick question ppl.My wife is visiting Qatar with our infant son and is considering getting a medical card for him since he's got a whole slew of vaccinations in the coming weeks.Question I have is (1) does everyone on a visitors visa need a medical card.The Mrs still has a valid resident visa,baby's ona visitor visa.They both have international medical coverage under my policy in the US
(2)Whats required to get a medical card-birth certificate etc.
