Health Centre Change in Health Card

You can change the Health Centre from one centre to another centre.

Steps:

Go to nearest centre(you want to change) and inform this to counter.

You have to fill the form with basic details (phone no,current address and Kharamma Electric no)

Immed. they will give the new ref no and centre no(fixed in the card)

Next renewal time new card will be issued.

For Centre # 21 to near family centre(any ) same procedure but you have to take the Mrs health Card..

Others remain same

My Electricity & Water are included in the rent, therefore I do not receive any electricity bill! what should I do to change my Health Card as they require copy of the electricity bill.
