doctors private practice vs hospitals/clinics

just wondering if all the good doctors can only be found in the clinics/hospitals, or do they also run their own private practices? it just seems difficult to book in and see the doctor you want if they are based in a hospital...is this usually the case?
my experience in europe was that doctors also have their own surgery (pediatricians also do house-calls/visits...does this happen in doha?) and then you can see them at the hospital they're associated with if the situation warrants.
what is the case in doha with doctors?
