may be you need to dial PUK code, call QTEL and inform them about your problem. they will provide your PUK code(unless its not stolen;)
otherwise go to nokia and format your phone(avoid local technicians). you will loose all the datas from your phone while formatting.

Try to reset it to factory setting by doing the following
Soft reset code *#7370#
Hard reset code *#7780#
(if the soft didn't help try the hard reset)
u will be asked for a security code
Enter 12345
If the hard format code mentioned above does not work, try the following manual method to hard reset/format Nokia N95:
Turn off the mobile device.
Press and Hold the following three buttons: the green Call Answer button, the “*” button, and the number “3″ button.
While holding these buttons, turn on the phone and keep holding the keys until the mobile phone restarts and prompts you to enter details.
