first inform qtel to block ur sim ,, and then go to the capital police station and file a case tat ur phone is stolen ,, so as soon as anyone tries to put another sim or uses ur phone ,, they will be tracked and caught!

If you are sure that it is stolen then better call Qtel service center and ask them to block your SIM otherwise someone can abuse your mobile by making expensive calls. I am not sure if reporting to police is of any use.

You need that IMEI number to brick the phone. The phone will be a useless brick if locked via the imei number.
If you dont know the IMEI and have the original purchase receipt. Visit the dealer to find the imei
