Residence visa for baby without birth certificate attestation

my baby born in UAE and from india i change her name. she got new passport with new name. i tryed get new birth certificate from india and its i failed get new birth certificate.according authority i have to visit UAE and to get new one..i got a name change affidivit from indian with attested by qatar embassy india. could you please tell me with affidivit i can apply for residence visa for my kid..please tell me what do
