NOTARIAL SERVICES/AUTHENTICATION/CERTIFICATIONS AT THE EMBASSY
For each document QR 100
Requirement for each type of consular service:
A. NOTARIALS- Special Power Attorney (SPA), Acknowledgement letters, affidavits or sworn statements.
1.) Personal appearance of the person executing the document.
2.) Copy of affiant's passport; and for affidavit of support:
3.) Salary certification and Certificate of No Objection to employee's sponsorship, to be issued by the affiant's employer.
B. AUTHENTICATION- Requirements for authentication depend on where the document to be authenticated by the embassy was originally issued.
1.) Documents from the Philippines- DFA authentication of the document.
2.) Documents from abroad (not from the Philippines) - Authentication of the Philippines foreign Service Post( Embassy or consulate) having jurisdiction over the document's country of origin.
3.) Documents from Qatar (including labor/employment-related document)-Authentication from the Ministry of Foreign Affairs(MFA) which requires the stamp of the Qatar Chamber of Commerce and Industry (QCCI) for documentsfrom the private sector or the validating stamp of the issuing Qatar government agency if the document is not original; If document is for hiring in the Philippines, it must also be verified by the Philippine Overseas Labor Office (POLO).
Note: Please follow the correct procedure for labor document needing Embassy authentication; QCCI or concerned Qatar government agency, then Qatar MFA,then POLO, then Philippine embassy.
C. CERTIFICATION- Requires document/s proving what is to be certified (e.g.drivers license)
Note: Certification to be issued by the Embassy are subject to the limits of its mandate/authority as designed by the law.
D. NBI Fingerprinting- for those without NBI clearance.
1.) Passport Copy of the applicant.
2.) Personal appearance.
3.) Notarization fee of QR 100.
Note: NBI Clearance is required by Qatar immigration for employment/change of employment purposes and for the issuance of the Qatar Police Clearance.
The fingerprinted NBI form notarized by the Embassy (or for those with expired NBI clearance, the expired NBI Clearance itself ) has to be sent by the applicant to the Philippines together with a Special Power of Attorney (SPA) for the person who will file the application form in the NBI.
