Few points answered here.

1. For me personally, they insisted for a rent agreement with my name on it. I do not know about the company accomodation. I guess you need to mention it in the NOC from your company stating they have provided so and so accomodation. Please check with others as I dont want to misguide.

2. Bank statement is not required for visit visa.
