Pearl Divers is the PADI place in town. You can find them on Al Mirqab (Al Nasr), about 100 metres 'East' (towards town centre) from Doha Clinic. Their number is 4449553. If you can remember your name and other personal details then you can get a replacement card :D
Diving with Pearl Divers or via one of the hotels isn't cheap. You might find that Doha Sub-Aqua Club is more convenient, cheaper and provides a better diving experience. It is very easy to do the 'crossover' course from PADI to BSAC.
Note (1) You might find that the Marine World shop (turn inland at Perfume R/A and turn right after the Museum, it is on your left) is often cheaper and has a better selection of kit than Pearl Divers ...
Note (2) I have got nothing against Pearl Divers; they're certainly friendly people - but they're not the only option around!
Note (3) - there is also a diving club in Al Khor; I don't know the contact details for them, although DSAC could put you in touch.
