Does Anyone have a template for a NOC for Saudi Transit Visa?

I wish to travel to UAE by car & require a saudi transit visa. The agents require a No Objection Certificate from the company addressed to the Saudi Embassy.

I asked my company HR and he asked me to bring a template. Could anyone provide a template or atleast provide me the details as to what should be the contents of the NOC for saudi transit.

Your help will be greatly appreciated
