Coronavirus/COVID-19 Consent & Questionnaire
You are being invited to participate in a research study to assess the knowledge and perceptions pertaining to the Coronavirus disease outbreak (COVID-19). Weill Cornell Medicine - Qatar is conducting this study. Participation in this survey is entirely voluntary. It will take approximately 3-5 minutes to answer the questions in this survey.

The survey is completely anonymous, and all answers will be kept strictly confidential. You have the right not to participate in this survey, and if you do decide to participate, you can change your mind or skip any question that you do not wish to answer. We do not foresee any risks or discomfort arising from participating in this survey. There are no direct benefits to you from participating in this survey. While it is hard to predict the spread of Coronavirus/COVID-19 to this region, it would be helpful to determine the knowledge and perceptions about Coronavirus /COVID-19 among residents in Qatar and the Middle East and North Africa region so preventative measures can be implemented as necessary.
Completing this survey confirms that you speak/understand English, are above 18 years of age, are able to consent for yourself and do not belong to the vulnerable populations of prisoners/pregnant women.
