Hamad Health Card for family

Dear Friends,

Can you please advise me what are the documents required for getting health card from Hamad, it's for my wife and daughter. I have the same which was provide by my company.

Is it possible to get it without rental contract and electricity bill? I don't have both this documents as we are staying with our relatives.

If any possibility, please advide. Thanks

Check with your HR dept , they will guide you on it ..................

Hi,

You need to produce a rental agreement contract under your name and it should be attested by the Baladiya else you need to have a utility bill like electricity bill, mobile bill. In addition to this you need passport and QID copies of your family to be submitted. 100 Riyals for each appliant.
