The company that has offered the job will be able to transfer from visit visa to work visa without exiting once they have the visa ready. but it also depends at that point of time whether the immigration instructs to exit and come back.
If they deny extending the visa explain to them that it is a matter of job and the company has requested few days beyond the expiry date to complete the process. Also keep trying as getting extension depends on which counter you end up in. Depends on the person sitting there. so keep trying. The fee for extension is around 200QAR per month i believe. but stay without proper extension will cause heavy penalty.
Also check with the company if you are unable to extend will it be ok if she travels back to home country and will they treat her as a local contract or external contract. Because for most companies external contracts are entitled to tickets.
I think you have got extension for 2 or 3 months at a time after medical. Now before expiry i.e prior 15 days just request them to extend more months, if not possible 1 month more. I hope they will do this definitely. Like this, you can extend upto 1+5 months.

I went renew visa after 3 months they said this is the last extend I request they said no .what I have to do I need more 1month
