how to make your health card?

AOA...lz can any one tell me how to make health card here?what documents needed?and how much time they will take to hand over it...what should I prefer if I ve Inurance card government hospital or private? JazakAllah

You need following documents

One photographs

Passport and RP copies

I.D card

Birth certificate for kids

Go to your nearest health centre get form and fill it.

Pay 100 reals

They will give you receipt with HMC No. Card will be ready in one week sometimes even more time it takes but you can use receipt with HMC No immediately until you get HMC Card
