Change Name Procedure for Philippine Passport Expatriates

1. Go to Phil Embassy and apply for new passport with new name (240qr). This will take 30 days. They will send you a message if your passport is ready to be claimed.

2. Request in Phil Embassy for Certificate of Change Name (100qr) ; You can request this if you have your new passport already; You will need to come back after 2 days to claim this.

3. Fill up an application form from (change name) http://www.moi.gov.qa/site/english/departments/PassportDept/sections/sec1083/1083.html

4. Go to your company's Immigration section and have it stamped and signed.

5. Proceed to Immigration in Gharaffa with the following documents..
       - form that you had stamped and signed from company
       - Certificate of change name from Philippine Embassy
       - 1 pc Passport Pic (blue background)
       - Marriage Cert
       - QID Copy
       - Visa Sticker Copy
       - New Passport Copy
       - Husband's QID Copy
       - Husband's VisaSticker Copy
       - Husband's Passport Copy

2. Go to GATE 2, SECOND FLOOR and go to the room that has a sign "COMITTEE INTO LOOKING TO CHANGE NAME REQUESTS"

3. Pass all requirements to the lady/gentleman on the counter. She will then give you a Yellow Stub that has your name written on it. You have to come back after three weeks. In my case, I went back after a month but they told me to come back after two more weeks. I submitted all my papers on September 1,2013 and went back on October 1,2013 but they still told me to come back after two weeks. Unfortunately for me, it was EID HOLIDAY so offices were closed for a week. I went back on October 21,2013.

4. I went back to immigration GATE 2, 2ND FLOOR. I presented the yellow stub to the guy and he gave me a form. This form is for GULF TIMES and has to be typed. I went to the typing center near the petrol station and paid 5QR.

5. I went back to immigration and they had it stamped. They instructed me to proceed to GULF TIMES.

6. I went to GULF TIMES immediately that day. The publication costs 100qr. The lady told me that it will be published the following day and I have to buy a newspaper and bring the whole newspaper page back to immigration. (Take note: do not cut the newspaper)

7. Secure a copy of the newspaper with the advertisement that you requested for. Might as well get extras, just to be sure.

8. Right after 15 days of your publication and no one has objected regarding your name change, go back to the immigration with a copy of the newspaper page.

9. Go back GATE 2, SECOND FLOOR and submit the neswpaper with your yellow stub. They might not tell you anything at this point but you have to go back after one week of newspaper submission. This will give them ample time to work on your papers.

10. After a week or so, go back to GATE TWO, SECOND FLOOR of MOI. Show your passport or your QID ask if your papers are ready. If they are, the man will print out a scanned document of the very first paper that you filled out. They will instruct you to go to GATE 1, FIRST FLOOR.

11. Proceed to GATE 1 with that paper, find the elevator and go to the FIRST FLOOR. Tell the gentleman on the counter that you are there for you change name and he will give you a number. Wait for your number to be called. Once you're number is called, hand over the paper and he will just write something on the blanks below.

12. Afterwhich, you have to go back to GATE 2, GROUND FLOOR. Get a number from the gentleman. Tell him you're there for name change he will give you a number. Wait for your turn. Once your number is called, hand over the paper along with your new passport with new name, QID and old passport. He will ask you for a debit/credit card. This costs 100qr. The man will print out your new visa sticker and place it in your new passport. You have to sit and wait so they could print out your new QID. When you already have your QID and Passport, make sure to double check your visa sticker for any errors like wrong spelling of your name, wrong QID number, wrong passport number. Because, this will be a problem if all documents are not the same.

13. DONE! You might just have to submit your new QID and Passport copies to your HR Department so they could update your file.

This procedure is for adult; for minor (below 15 yrs) no need to put adv in Newspaper

I surprised they didn't ask your marriage cert.in Arabic cos they did to me.so have to come back again.take note marriage cert with stamp from MOFA.

I just did mine today.but giving me hard time too many requirements.i was frustrated and disappointed because I book my flight already last month.and had to cancel today because I don't know when will this be finish transferring rp to my new passport.i wish I read all of this before I change my family name because I don't have any idea that this will take a long time to process.

hi, just wanted to ask, which one should go first? Do we need the visa to be stamped on the passport first or we need to do the changing of names and stamping of the visa simultaneously? Cause my old passport with visa stamped in it will be expiring on the 31st of March, and base on your experience it will be taking minimum of 2-3 months, I am just worried that my old passport will be expired when I finish the whole process..

@yayoi, the letter stating your reason for change name which you had from the petrol station typing center was signed by your sponsor? because for me I was the one who signed the letter and the officer at the change name office just accepted it. Only the form from moi website was the only form signed by my sponsor. I submitted my application last March 4, 2014 and up until last March 26, 2014, when I went to follow up the officer said "not yet ready". I am now confused, does it really takes that long? Thanks for any information.

@patrene, i went to immigration ysterdy, one of the requirements is MC with stamp from Minstry of Foreign Affairs.. shall i go to MFA directly or they need other requirements along with my MC.. i was married here in phil embassy, am i right to assume that my MC doesnt need authentication..?? pls help..
