fill the form provided by nha,
doc required:
1-green card (immunization card) original and two photocopies, front and back,
2_mothers passport copy one each, (front, last, and residency page)
3-fathers passport copy one each, (front, last, and residency page).
4-submit it on ground floor, (Receiving form counter), they will return it back
5-go to same building 3rd floor, for checking spelling, etc.(right turn from lift last door on right),
6-after checking, again ground floor same counter, take receipt to pay (20 riyal for 1 piece of certificate),
7-cashier, on right side ground floor,
receiving certifiacte
1-go after 3 or 4 working days, ground floor,
2- if not ready, go to 3rd floor, same person as before, again check spelling urself,
3-go to Director office, same floor,get it signed
4-again go to ground floor, get 2 stamped and 2 documents (one original and one copy
