Do they need application form for visa extension

Yesterday,i took my mother to medical commission for her visit visa extension.

But i found that the system was changed.Finally I got confused.So i have few question about the procedure.

1.My wife wait a long queue for typist to type the visa extension application form.but nobody take it in the end.

2.No required photos now?(previously it was required)
3.No need the copy of the one-month visit visa?
4.People told me I can go to immigration directly after 3 days.If my mother pass the test,the result will be online.if not her result will not be online.
