For me, whether there is an HIV issue or not, the medical should be a part of yearly medical check-up for expats (why expats), because I am an expat. Let the locals be taken care of their own government whilethe expats be taken care of by the concerned company or goernment agency that handles expats.
I am not focussing on HIV or whatever diseases may be arising so fast in this country or region.
In my country, a lot of company are already requiring the mandatory medical test, ANNUALLY, for their regular employees as an added benefit (of sort) to the employee. And good for me, I am always employed in such companies!
