HI QL friends,

how to change the name for new born baby birth certificate in Qatar. I want to add a surname, the child was born at HMC. if anyone knows a complete procedure, please help me.

First Visit HMC Birth Registration office (From where you applied birth certificate previously), tell them your requirement and you will get a application from them. fill it with supporting documents and submit it with SUPREME COUNCIL OF HEALTH. thats it.

For more details please ask the counter staff at HMC During your vist.
