My wife is currently here in Qatar with a tourist visa which is valid only for 1-month stay. Is it possible for me to apply for her a family visit visa without exiting or leaving Qatar before her current tourist visa expires? Appreciate anyone's input and help. Thank you in advance.

Yes, it is possible to obtain family visit visa without leaving Qatar while on tourist visa.

You may get it extended for 6 months after getting the medical done. but after that either you get her RP or she would have to exit the country and if all's okay come back again.
