Tax Card ... Need Help

Can any one tell me from where I can issue my Companies Tax Card ? And pls tell me the location aswell. Need it pretty urgently . So help me out people. Pm me or reply here as I am pretty much dpendng on Ql for positive results. ThanK you ..
