(don't know exactly the location). just fill out the form, provide qatar id and passport copies of you and your wife. just pay qr20 each copy. but if i were you, i'll get 2 copies. then proceed to phil. embassy, fill out the new passport application form, bring atleast 6 copies of your passport, qatar id, authenticated marriage contract and birth certificate of your child. new passport and registration fee will cost you qr364. normally it (new passport) will take 45 days to be released but i got mine in just 20 days.HTH...
