Nanny Visa

Does anyone know the procedure for applying for a work visa for our nanny? She was here on a visit visa that has now expired. We are aware that we will have to pay a penalty for the expired visa, but we also wanted to know what are steps to apply for a work visa after the penalty has been paid. Any information would be greatly appreciated. Has anyone else been through this before?
